What Do I Need to Provide If I’m Based in California vs. Outside California?
All applicants should provide their business name, contact information, email address, physical address, and business-related documents, including a business license, sales tax certificate, and tax ID. California-based resellers must also complete the California Resale Certificate (Form CDTFA-230) and submit it to sales@solsticemed.com or fax it to (323) 221-2448.
Resellers outside of California should send a copy of their State Sales Tax Exemption Certificate to the same email or fax number.
